I had massive problems too. In the end, it was dovecot's problem:
a combination of
http://www.dovecot.org/list/dovecot/2007-August/024840.html
http://www.dovecot.org/list/dovecot/2007-August/024842.html. Now
that I delete the index cache daily, I can sync a 100'000 message
Maildir in a little less than 2 minutes.

Never had that happen.
